Root Cause Analysis — a local workshop for clinical staff

Benefits of the workshop

Clinical teams have to be able to analyse adverse events or near misses that occur in the delivery of patient care as part of an effective clinical risk management programme.

Some clinical groups have developed an open and inquiring attitude toward the analysis of events that harm or could harm patients or staff. But clinical teams and clinical service managers often benefit from learning a structured approach to carrying out root cause analysis.

Intended participants

The workshop is designed for clinical staff and managers of clinical services who have responsibility for leading and for participating in clinical risk management in their services.

Clinical risk managers and clinical governance facilitators will benefit from participation in the workshop.

Learning objectives

The workshop helps clinical staff and managers understand the importance of systematically analysing adverse events and near misses that occur to patients in healthcare settings in a way that minimizes threat to the staff involved. Participants learn:

  • key concepts and terms involved in root cause analysis
  • international evidence on the incidence of adverse events in healthcare organizations and root cause analysis
  • benefits and drawbacks of carrying out root cause analysis in a systematic way
  • how to carry out root cause analysis in detail using a systems approach, represented by the acronym S-O-L-V-E-D
  • practical tools and techniques that can be used to carry out root cause analysis
  • how to put together a team to carry out the root cause analysis
  • how to organize the work that is involved in a root cause analysis
  • how to document formally a root cause analysis.
Venue and workshop format

To involve as many clinical staff as possible, we offer the workshop at a local level on a day most convenient to the participants. We travel to your organization to provide the workshop.

The workshop includes presentations of principles and group work. Small groups carry out all the stages of a root cause analysis, using an actual adverse event, practising using a number of practical root cause analysis tools, and making decisions about how a root cause analysis might be carried out in their clinical teams. The workshop starts at 9.15 am and ends at 4.30 pm.

Materials

Each participant receives a copy of a 76–page book that includes principles, step-by-step guidance, examples and practical assignments to do in the workshop. Certificates of participation in the workshop are provided.
